MyHealth Centre is now WELL Health Diagnostic Centre in Mississauga! We're the same team with a new name, located in the Credit Valley Professional Building beside the hospital at the intersection of Erin Mills Parkway and Eglinton Avenue West. We provide OHIP-covered ultrasound, X-ray, mammography and bone mineral density services - with shorter wait times, faster report turn-around, and the highest standard of patient care. Step 1: Requisition
Download a requisition and obtain a signed copy from your referring healthcare provider*. If possible, E-mail or fax the signed requisition to our clinic.
Step 2: Schedule
All of our services require a scheduled appointment, except X-ray, which is provided on a walk-in basis**. Please schedule your appointment by telephone, or live chat located in the bottom-right corner of this website.
Step 3: Prepare
Please review our Test Preparation Guide prior to your appointment. Bring your Ontario Health Card (OHIP) and requisition to your appointment and arrive 15 minutes early to register.***
*Our OHIP-covered services require a signed requisition from a certified Ontario healthcare provider, except breast screenings for women ages 40-74. If you need a requisition, you can visit a family doctor, walk-in clinic or community health centre.
*Already have a doctor’s requisition? We accept signed requisitions from any certified Ontario healthcare facility, including hospitals and independent health facilities.

***If you have mobility concerns, please ask your caregiver to accompany and assist you.
We will send your diagnostic report to your referring healthcare provider, who will follow-up with you. We can send it to additional healthcare providers upon your request.
