MyHealth Centre is now WELL Health Diagnostic Centre in Sarnia! We're the same team with a new name, located in the Bluewater Medical Clinic, across the street from the hospital. Please enter through the doors located on the right side of the building and take the elevator or stairs down to the basement. We provide OHIP-covered cardiology, ultrasound, X-ray, mammography, and bone density services - with shorter wait times, faster report turnaround, and the highest standard of patient care. Please call or chat live to request your appointment, except X-ray, which is provided on a walk-in basis. We look forward to seeing you!

Step 1: Requisition
Download a requisition and obtain a signed copy from your referring healthcare provider*. If possible, E-mail or fax the signed requisition to our clinic.
Step 2: Schedule
All of our services require a scheduled appointment, except X-ray, which is provided on a walk-in basis**. Please schedule your appointment by telephone, or live chat located in the bottom-right corner of this website.
Step 3: Prepare
Please review our Test Preparation Guide prior to your appointment. Bring your Ontario Health Card (OHIP) and requisition to your appointment and arrive 15 minutes early to register.***
*Our OHIP-covered services require a signed requisition from a certified Ontario healthcare provider, except breast screenings for women ages 40-74. If you need a requisition, you can visit a family doctor, walk-in clinic or community health centre.
*Already have a doctor’s requisition? We accept signed requisitions from any certified Ontario healthcare facility, including hospitals and independent health facilities.

***If you have mobility concerns, please ask your caregiver to accompany and assist you.
We will send your diagnostic report to your referring healthcare provider, who will follow-up with you. We can send it to additional healthcare providers upon your request.
